adjunct Case ManagerWe are currently seeking a highly motivated adjunct Case Manager to join our team in Hawthorne, NY.The ideal candidate will have an Associate's degree in Healthcare, Social Work, or Psychology, or a valid New York State LPN license. The adjunct Case Manager will work Monday to Friday during the day shift, from 8 am to 4 pm, and will be compensated at a rate of $25-$30 per hour. This is an in-person position only; no remote work will be considered. adjunct Case Manager Qualifications required » Associate's degree in Healthcare, Social Work, or Psychology OR a valid New York State LPN license » 1 year of practice in case management, healthcare coordination, or a related field preferred » Excellent communication and interpersonal expertise » Strong organizational and time management expertise » capability to work independently and as part of a team » Proficient with Microsoft Office and electronic medical record systems » capability to travel to various locations as needed adjunct Case Manager Responsibilities: » Assist the Case Manager in managing a caseload of clients with complex medical and social needs » Coordinate services and resources to meet the needs of each client » Help clients navigate the healthcare system and access appropriate services » Work collaboratively with healthcare providers, social service agencies, and community organizations to ensure continuity of care » Ensure that clients receive timely and appropriate care, and follow up to ensure that care goals are met » Document all interactions with clients and providers in conformity with established policies and procedures » Participate in case conferences and meetings with other healthcare providers as needed » Perform other tasks as assigned by the Case Manager We offer a competitive income and opportunities for professional growth and development.
