CORE Peer Specialist PT

JOB DESCRIPTION

Position Summary: Empowerment Services (Peer Support) are non-clinical, peer-delivered services with focus on rehabilitation, recovery, and resilience. They are designed to promote expertise for coping with and managing behavioral health symptoms while facilitating the utilization of natural supports and community resources. These services are part of Community Oriented Recovery Empowerment services (CORE). These services are for individuals with a Serious Mental Illness that also have a Health and Recovery Plan (HARP) through their Medicaid Insurance.
Peer Support must include the identified goals or objectives in the member's individualized service plan, with interventions tailored to the individual. These goals should promote utilization of natural supports and community services, supporting the person's recovery and enhancing the quality of their personal and family life. The intentional, goal-directed activities provided by this service emphasize the opportunity for peers to model expertise and strategies necessary for recovery, thereby developing the individual's expertise and self-efficacy. These services are provided through the perspective of a shared personal practice of recovery, enhancing the individual's sense of empowerment and hope.
As a Peer Specialist, you will be providing services in the community to individuals all over Ulster County.

Responsibilities Include:
   » Meeting with individuals 1 on 1 in the community or at their homes
   » Work with individuals on your caseload to develop person-centered goals in an Individual Service Plan
   » Assist individuals on your caseload with achieving their desired goals
   » Document as required in the HER
   » Complete all required trainings
   » Participate in team meetings as scheduled
   » Be comfortable working with individuals with a serious mental illness in all stages of recovery

Health, Safety, & Compliance
   » Maintain a work environment in a clean, organized, and safe manner. Conduct everyday inspections of work areas to look for unsafe conditions
   » Use all required safety devices and personal protective equipment.
   » Ensure all accidents are reported to manager and Human Resources.
   » Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.
Agency Standards
   » Demonstrate positive work attitudes and habits. Act as a function model for vocational success for individuals
   » Maintain a clean and neat appearance consistent with agency standards and act as a function model
   » Attend all required meetings and trainings
   » Participate in training programs for professional growth as scheduled and approved
   » Responsible for knowledge and adhering to Gateway Hudson Valley's Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies. Keep abreast of any changes or modifications to policies or regulations. Ensure that all staff are knowledgeable of applicable regulations, laws, and policies
   » Perform other tasks as assigned

Qualifications

REQUIRED QUALIFICATIONS
suitable education
   » High School Diploma

practice:
   » 1-3 years practice working with individuals with serious mental illness and/or substance use disorder
   » Lived practice
Required Certifications:
candidates must be certified in one of the following:
New York State Certified Peer Specialist, can be provisional (NYCPS) or
New York Certified Recovery Peer Advocate, can be provisional (CRPA)

WORKING CONDITIONS
   » Must be able to Sit or stand for 8 hours/day
   » Must be able to bend down or kneel occasionally (1-2 times/week)
   » Must be able to use hands, wrists, and fingers with full range of motion
   » May occasionally be required to lift no more than 25 pounds (1-2 times per month)

REGULATORY CLEARANCES REQUIRED:
   » Must have a valid NYS Driver's License and maintain a driving record which meets the insurable criteria of the Agency's automobile insurance carrier
   » Driver's License Abstract
   » Must have a favorable result from all state, federal, and agency mandated background checks
   » New York State Justice Center Fingerprinting Criminal Background Check
   » OWPDD MHL Abuse/Neglect History Check
   » Statewide Central Registry Check
