Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. we At Presenthave the need for a WIC Qualified Nutritionist at our Poughkeepsie location.The WIC Qualified Nutritionist would be responsible for providing WIC Registration Nutrition assessments and counseling staff of the WIC (Women, Infants, and Children) program at the assigned Sun River Health site(s). They will be responsible for providing nutrition education for individuals and groups. Performs job tasks in compliance with systems and job specific standards. Essential Job tasks and Responsibilities: » Provides nutrition counseling to patients referred by clinicians » Performs initial nutrition history on each patient » Develops and implements a care plan located on the assessment of nutritional and health needs » Provides instructions on a normal or special diet » Schedules follow-up appointments and evaluates the reaching of goals » Provides nutrition counseling for diabetic patients as part of diabetic care plan » Provides nutritional education materials to patients to support counseling efforts » Makes referrals to other health care providers and community services as appropriate » Counsels individuals and families on nutrition and diet located on the particular needs of the individual or family as determined by their nutritional and health risk » Attends staff meetings and in-service training sessions at both local and state levels » Provides consultation into the development of program policies and procedures » Provides direction and executes health education programs as needed » Must demonstrate expertise and skill necessary to provide care appropriate to the age of the patients served » Must demonstrate expertise of the principles of grown and development over the life span Minimum Education Requirement: Bachelor's Degree Related Work practice: 2-4 years Preferred Education Requirement: Master's Degree Preferred Work practice: 5-8 years Required Licensure: Valid CPR card from American Red Cross or American Heart Association Specialized expertise & expertise: » capability to communicate in English, both orally and in writing » capability to operate a motor vehicle and travel within the Sun River Health service site area » expertise of nutrition principles as well as expertise of nutrition during pregnancy and lactation, as well as infant and child feeding practices » expertise of the geographic community serviced and resources » Competence in nutrition assessment counseling » capability to collaborate with other member of Hudson River HealthCare staff in order to maintain high quality standards and achieve client satisfaction income Range: $39,130.00 - $42,770.00 annually Skills:
