Social Worker - Home CareOverviewArchCare at Home is growing in our brand new location in the Kingston Plaza! We are now seeking a Social Worker - MSW for the Ulster County Region . This opportunity is a Fee for Service position. ArchCare at Home is a certified nonprofit home health agency that delivers the highest quality care to patients of all faiths in the comfort of their own homes. Our service area includes the five boroughs of New York City and the Hudson Valley.The Social Worker is responsible for the provision of professional social work services to patients and their families and provides social work consultation on patient care to staff in compliance with applicable laws, regulations and Agency policies.ResponsibilitiesThis Fee For Service Social Worker - MSW opportunity in Ulster County carries out the agencys mission, philosophy, goals and objectives within guidelines of Agency policy and position function.» Interprets and implements the Agencys philosophy to staff and members of the community. Gathers pertinent social information from patient/family/staff or from patient record on referred patients. » Assesses the environment of the home and community to identify elements which impact negatively on the patient/family capability to function. » Provides concrete social work services to enable patients to receive entitlements. » Provides direct short-term counseling services to patient/family or makes appropriate community referrals. » Teaches coping expertise to patients/families to assist them in their adjustment to the life changes connected with illness/discapability. » Guides patients/families to accept responsibility for their own actions, to understand that their actions/inactions have a consequence. » Guides patients and families to adopt self-help techniques to cope with physical, emotional and environmental health problems. » Recognizes attitudes and cultural patterns that impact on health. » Helps patients and families to develop capabilities that permit them to make optimum use of available health and social facilities. » Participates in conferencing and planning with other disciplines caring for the patient. » Transfers cases to the appropriate social agency if the need for continued service exists at the point of termination of the CHHA/LTHHCP social work services. » Prepares clinical progress notes and documents in the patients record, as indicated. » Available for planned social work consultation on patient care with staff, as needed. » Attends and participates in staff meetings and case conferences, as needed. » Performs all other related tasks, as required. » Participates in Performance Improvement activities. » Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan. QualificationsThe qualifications for this opportunity in Ulster County include:» Self direction with capability to work with supervision. » Is flexible and cooperative in fulfilling function obligations. » Current drivers license, required insurance and car available for work, as required. » The canididate must have a Masters Degree in Social Work from an accredited school. » A minimum of two years of social work practice in the health care setting as a member of a multidisciplinary team. » Possesses and maintains good physical stamina and mental health, including satisfactory health clearance as required by NYS regulations and Agency Policy.
