Service Coordinator - Human Services (Direct Hire)Service Coordinator – Human Services (Direct Hire)Job location: New City, NY Schedule: Full-Time income: $60,000/year Type of employment: Direct Hire | In-Field & Community-located A well-established human services organization is seeking a dedicated and compassionate Service Coordinator to support participants in the Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD ) waiver programs. This direct hire opportunity is ideal for professionals who are committed to helping individuals live independently while navigating essential community-located services. Service Coordinator Responsibilities: » Create and implement individualized service plans (ISP/RSP) located on participant needs and preferences » Coordinate Medicaid waiver services, healthcare access, and community supports » Conduct regular in-person visits, including required quarterly home visits » Maintain program eligibility by securing initial and annual level of care assessments » Schedule and lead interdisciplinary team meetings to support care planning » Respond promptly to participant emergencies and resolve service delivery issues » Maintain accurate, confidential records in compliance with state regulations Service Coordinator Qualifications required Required Education & practice: »Master’s Degree in Social Work, Psychology, or a related field with at least 1 year of significant practice OR »Bachelor’s Degree with 3 years of practice in service coordination, case management, or resource navigation for individuals with disabilities or seniors Additional General Requirements: » Valid NYS driver’s license and access to a reliable, insured vehicle » Strong interpersonal and organizational expertise » Proficiency in Microsoft Office and web-located systems » Working expertise of HCBS waiver programs is highly preferred » Bilingual (Spanish-English or Creole-English) is a plus What You’ll Gain: » A reliable full-time position with a significant impact » Opportunities to work independently while supporting vulnerable individuals » A chance to grow your career in the human services or healthcare coordination field » The flexibility of field-located work paired with administrative responsibilities Apply today to make a difference in the lives of individuals striving to live independently in their communities.
