Intake Coordinator - Regional Peer Services

As an Intake Coordinator, you'll provide information and referral services with resources and options that are necessary in making informed choices about living, learning, and working independently.

Independent Living, Inc., is a progressive and dynamic cross-discapability services organization and a leader in the provision of peer services in the Hudson Valley region. As an Intake Coordinator - Regional Services, you must have excellent written and verbal communication and attention to detail expertise. Must have the capability to multitask, engage and assess the needs of individuals, as well as computer practice with various programs and databases. The minimum educational requirement is a High School Diploma or equivalent. Bachelor's Degree, preferred. CRPA and/or NYCPS certifications, preferred.Must have lived practice with substance use disorder and/or mental health disorders and a availability to disclose that practice when necessary.

   » Supports the following programs: Onward Recovery & Reach One in the following counties - Orange, Rockland, Sullivan, Ulster, Dutchess, & Westchester, Putnam.
   » Provides excellent customer service and support and creates a welcoming front office environment.
   » Greets, engages and assesses individuals' needs for Independent Living (IL) services, both in person and via the phone. Responsible for triage of consumers. Routes calls and walk-ins to appropriate staff and handles scheduling.
   » Initiates completion of intake forms; compiles intake packets (Reach One, Onward Recovery, PeerRx, Group) that include intake form, CSR documents, and HIPAA forms and gives them to appropriate staff members with new referrals.
   » Maintains a list of all program rosters and distributes new referrals to the appropriate staff members.
   » Schedules an initial visit for all new consumers with the appropriate staff member. Is responsible for rescheduling appointments if a staff member is out unexpectedly.
   » Orients new RCOC members to the center.
   » Performs HCBS/HARP eligibility checks in epaces twice a month for all ILI Adult BH HCBS consumers and reports the results to the Regional Director of Peer Services.
   » Ensures that all vouchers are scanned and filed in the appropriate program's i-drive folders.
   » Accompanies other staff members in the field when transporting a consumer to treatment.
   » Facilitates support groups and sober social activities as needed.
   » Compiles and distributes the monthly RCOC calendar. Creates flyers for all calendar events, groups, & activities.
   » Maintains an email distribution list to communicate RCOC events, calendars, activities, and announcements.
   » Assists and serves as a mentor for RCOC volunteers and interns. Maintains volunteer/intern records and enters corresponding data into the appropriate data form in the agency database.
   » Makes appropriate connections/linkages to internal and external resources and enters corresponding service referrals in the agency database.
   » Provides short term core IL services that promote personal growth, self-advocacy and empowerment.
   » Develops a deep knowledge of current and emerging resources (also known as independent living resources - ilr) and federal and state discapability rights laws.
   » Enters service documentation for all services directly provided by the Intake Coordinator in all programs (Reach One and Onward Recovery - all counties).
   » Provides information and referral services and assist individuals in making necessary informed choices about full inclusion of people with disabilities.
   » Provides individual and systems advocacy.
   » Provides a set of core services geared toward promoting self-help, equal access, peer function modeling, personal growth and empowerment.
   » Provides counseling, time and attention, empathy, validation, someone to confide in and trust, guidance and modeling of effective community living on an as-needed basis, all over the period of transition. Share ideas and practices about living with a discapability in order to gain greater consciousness and control over one's own life.
   » Teaches independent living expertise that help people become self-sufficient in areas such as housing, travel transportation, personal self-management, employment, financial management and self-advocacy.
   » Offers employment readiness services to support consumers in their efforts to prepare to enter or re-enter the workforce (i.e. help with preparing resumes and completing job applications).
   » Assists consumers with access to treatment (inpatient, outpatient, detox, MAT). This includes calling facilities and inquiring about availcapability of beds, insurance, & eligibility requirements.
   » Records minutes and agreements for all staff meetings and stores them in the appropriate program folder on the i-drive.
   » Maintains the i-drive program folders including all forms, calendars, vouchers, staff meeting minutes, etc.
   » Maintains the upkeep of front office operations and processes.
   » Answers all incoming calls using proper phone etiquette and excellent communication expertise. Forwards all calls to the appropriate staff member with a polite and cheerful attitude.
   » Ensures front desk and center coverage at all times, coordinates with program managers and schedules staff as needed.
   » Maintains inventory of ALL supplies and orders as needed.
   » Facilitates operation of all office equipment including but not limited to computer technology, telephone system, fax machines, copiers, etc. Orders service when necessary.
   » Responsible for maintaining everyday sign in and vehicle logs, sending all logs to the Chief Operating agent monthly.
   » Monitors and enforces Employee Sign In and Sign Out logs and Visitor Sign In sheet on a everyday basis and speaks to people who are non-compliant. Enters visitor data into appropriate data forms in the agency database.
   » Responsible for enforcing, monitoring and collecting monies for the Jeans Day Fund.
   » Ensures compliance of vehicle schedules, lists of approved drivers, keys and vouchers for gas card purchases.
   » Schedules and maintains vehicles and conference room reservations in Outlook as per employee requests.
   » Maintains employee contact listing in Outlook.
   » Attends and actively participate in all meetings and training sessions provided by ILI and/or other agencies as requested by the manager.
   » Processes ALL intakes, admissions, & discharges into the agency database.
   » Creates social media events and posts to communicate RCOC activities/outings/groups. Responds to social media inquiries.
   » Maintains office management files, including program manuals and RCOC resource binders.
   » Assists with training new employees with the use of the agency database and everyday operations.

Requirements
Qualifications required
   » High School Diploma or equivalent. Bachelor's Degree, preferred.
   » CRPA and/or NYCPS certifications, preferred.
   » Participate in offered trainings.
   » Honesty, respect for diversity, high personal standards, language and behaviors reflecting such values and the Independent Living philosophy.
   » Demonstrated capability to recognize the need for and facilitate connections between consumers and IL and other discapability related services.
   » Knowledge of local, statewide and national discapability related issues and community dynamics.
   » Excellent written and verbal presentation expertise.
   » Lived practice with substance use disorder and/or mental health disorders and a availability to disclose that practice when necessary.
   » availability to complete all trainings necessary to acquire CRPA and/or NYCPS credentials, including continuing education requirements to maintain the certifications.
   » Proficient in the use of Microsoft Office suite.
   » Bilingual; fluent in both Spanish and English.
