Job Opportunity: Intake Coordinator:Job description:
Intake CoordinatorOverviewthis job offer coordinates referrals received from Liaisons, Account Executives and through the Wellsky Portal. All referrals are reviewed for completion and uploaded to the home care EMR. The intake coordinator may also enter referrals into the EMR, completing all aspects including general demographic data, physician diagnosis, and referral source data to complete the referral. The position all may include checking eligibility and initial authorization from Managed Care Organizations. this job offer may require communication to the home care team, referral source and potential patients and their families.Responsibilities» Data enters patient information into Patient Information in the patient» Uploads data from the eFax and scans other documents related to the referral into the patient » Ensure all data fields are complete to ensure timely follow up with referrals, so they are entered the day before admission whenever » Check eligibility of patient insurance and documents that in a coordination note, indicating co-pay, co- insurance, and deductibles for the patient » Obtain initial authorization as appropriate and document coordination note to authorizations obtain if required or that authorization is not » Answers calls from referral sources and return calls within 2 hours or within the day of the » Teamwork: Collaborates with Liaisons, Account Executives and internal referral sources to ensure that all aspects of the referral and information is obtained and documented in the patient » Develops relationships and enhances appreciation for the needs to meet specified deadlines and the consequences of not meeting » Communication: Takes phone calls from care members, patients, caregivers, referral sources and others.
