Residential Case MgrJOB DESCRIPTIONAs a Case Manager, you will support clients in the Residential System and perform tasks in collaboration with a clinician. You will report to the Case Management manager, Residential and you will work onsite with community-located travel..Responsibilities: » Arrange and attend home visits » Prepare transition plans » Coordinate Independent Living Services » Be the primary contact between Schools and Residential » Coach family visits » Complete various assessments, including FASP and SPR » Complete accurate case notes and input the notes into Connections database » Provide transportation for clients » Maintain contact with DSS and family » Function as a contributing member of the interdisciplinary treatment team » Attend dorm meetings » Attend treatment review meetings Requirements and suitable education » Requires a Bachelor's degree or commensurate level of education and related practice. » practice with individuals in care systems preferred. » Must pass physical.
