Child Health Home Care managerPOSITION OVERVIEWAs a Children's Health Home Care Manager/manager, you will engage children, youth, and families via outreach and enrollment activities for prosperous enrollment in a children's health home and the provision of health home core services. You will report to the Director of Health Homes and you'll work on-site with some travel required.Responsibilities: » Engage children, youth and families through outreach and enrollment activities for prosperous enrollment in a children's health home. » Gather assessments and enter documentation that confirms eligibility and identifies area requiring support services. » Work w/children, youth, families and service providers to create a comprehensive Plan of Health Care. » Identify and engage additional providers ( e.g. natural supports, parent advocates, primary care physicians') with children youth and families. » Coordinate the continuing involvement of families and service providers in the execution of the plan. » Convene and conduct quarterly meetings to review progress updating Plan of Care, identifying progress and confirming continuing eligibility. » Document progress and arrange for eventual transitions between systems, placements, health homes, and multiple levels of care following changes in eligibility. » Use state and health home information systems to input required health home documentation.
