Lead Health Home Plus Care Manager

JOB DESCRIPTION

Full TIme- Hourly Position
$23/hour
Medical, Vision Dental, Generous PTO, 403(b), Life
POSITION SUMMARY
Health Home Plus (HH+) is an intensive care management program established to provide HH members the intensive services needed to stabilize their health and social service needs in the community.TheHealth Home Plus Care Manager is responsible for the provision of intensive Care Management services to individuals with Severe and Persistent Mental Illness in Ulster County. The Care Manager collaborates with mental health professionals in Ulster County and Managed Care Organizations to connect with and refer recipients of service with vital resources in the community to be prosperous in reaching goals.
Responsible for outreach and intake process for new referrals, including determining eligibility for program
   » Responsible for assigning cases to care managers in conjunction of director
   » Completes HCBS/CORE education with new intakes
   » Responsible for running monthly and quarterly reports for program as required
   » Monitor program quality and documentation compliance through regular review of case records. Make recommendations and implement changes for improvement in program quality
   » Evaluate program results and achievement of productivity, revenue generation and strategic goals. Collect and review monthly statistical reports; submit quarterly reports to the Vice President of Vocational & Therapeutic Services
   » Performs yearly eligibility assessment for services for each individual served and updates as needed
   » Provides technical assistance to other case managers with EHR
   » Uploads all necessary documentation upon intake and completes CM assignment
   » Performs quarterly chart audits at the instruction of the Lead Health Home
   » Maintains a small caseload of 8-10 health home plus clients
   » Provide services to clients in the community including referral to services, assisting with transportation issues, covering their social needs goals, and completing assessments and care plans
   » Reviews other CM's monthly contacts and ensures requirements are being met
   » Completes all required trainings
   » Enters new referrals and updates into MAPP system
   » Maintains timely documentation in significant EHR
   » Attend weekly staff meeting and supervision
   » Participate in county collaboration meetings as needed
   » Attend treatment team meetings as needed
   » Provide person-centered, trauma informed case management services
   » Act as the first point of contact for the lead health home and work with them on Quality Assurance and Compliance issues within our program
   » Work with CM's to ensure timely completion of care plans, assessments, and other required documentation
   » Provide training and support to other CM's
   » Excellent time management and organizational expertise

Qualifications

General Requirements:
   » Bachelor's in Psychology or related field
   » 3-5 years of practice working with individuals with a serious mental illness
   » At least 1 year practice working with Health Home Plus
Preferred Qualifications:
practice working with those with a Substance Use Disorder
practice with EPIC and MAPP

   » Maintain a work environment in a clean, organized, and safe manner. Conduct everyday inspections of work areas to look for unsafe conditions
   » Use all required safety devices and personal protective equipment.
   » Ensure all accidents are reported to manager and Human Resources.
   » Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.

Keep abreast of any changes or modifications to policies or regulations. Ensure that all staff are knowledgeable of applicable regulations, laws, and policies
   » Perform other tasks as assigned

WORKING CONDITIONS
   » Must be able to Sit or stand for 8 hours/day
   » Must be able to bend down or kneel occasionally (1-2 times/week)
   » Must be able to use hands, wrists, and fingers with full range of motion
   » May occasionally be required to lift no more than 25 pounds (1-2 times per month)

REGULATORY CLEARANCES REQUIRED:
   » Must have a valid NYS Driver's License and maintain a driving record which meets the insurable criteria of the Agency's automobile insurance carrier
   » Driver's License Abstract
   » Must have a favorable result from all state, federal, and agency mandated background checks
   » New York State Justice Center Fingerprinting Criminal Background Check
   » Statewide Central Registry Check
