POSITION OVERVIEW

Community Service Provider - Family Peer

A prosperous candidate for this job offer would be a creative and enthusiastic individual who is committed to helping children with emotional and mental health challenges through support and advocacy with parents and caregivers. The function requires local travel to provide home and community-located work with caregivers in their communities providing a core of services to ensure success. This includes the services of Peer Support and Caregiver/Family Advocacy and Support.
Northern Rivers is seeking a Family Peer to work with families in Schenectady, Albany and Saratoga Counties. Family Peers have identified lived practice as a caregiver or parent of a youth with behavioral health, mental health, substance abuse, or other special needs.
Essential tasks and Responsibilities
   » Providing support, advocacy and resources to parents located on the child's needs as identified in the Treatment Plan;
   » Coordinating with the caregiver on the scheduling and provision of services;
   » Working with family members to identify goals and objectives of treatment;
   » Facilitating service provision and attending significant meetings as required;
   » Completing all documentation as required.
Education and/or practice
   » Family Peers have identified lived practice.
   » Provisional Family Peer Credential required, with Professional Family Peer Credential to be obtained within 18 months.
   » Minimum of HS Diploma or equivalency.
   » Valid NYS Driver's License required
