Intake SpecJOB DESCRIPTIONPOSITION SUMMARYthis job offer is responsible for communicating with individuals within the community who are seeking Parsons OMH residential services. this job offer will gather information, make referrals, and complete paperwork for youth and families coming into program. ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned. » Receive initial phone inquiries and written referrals. » Gather pertinent information as required by program in order to determine eligibility for services requested. » Refer to alternate service providers as requested or needed. » Gather and enter information into database including referral information, demographic data, change of status, etc. » Meet with clients/families as needed to communicate significant information regarding Parsons' services and to receive signatures on releases of information and intake paperwork. » Prepare and distribute clinical record, admission paperwork and other documents for programs. » Develop general expertise of all Parsons' services in order to meet the needs of callers, including in emergency situations. » Schedule intake and admissions in collaboration with family and program » Other tasks as assigned.
