Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County.this job offer will support the following Sun River Health locations; Bedford, Church Avenue, Inwood, Westchester Square, the HUB, and Fountain House. SUMMARY OF POSITION: The Behavioral Health Engagement Specialist will be responsible for assertive engagement of BH patients who are lost to care, or who are at risk of becoming lost to care. ESSENTIAL FUNCTIONS: » Conduct outreach through live calls or mail to locate patients who may need additional support and services to retain patient in care. » Document and maintain clinical records in a manner that assures compliance with all agency policy and procedures. » Assist outreached patients with prompt follow up appointments to efficiently engage them in care. » Refer and connect patients to appropriate service providers » Coordinate patient care within interdisciplinary team. » Display professional courtesy at all times and maintains customer services attitudes. » Demonstrates capability to communicate effectively in a variety of circumstances. » Manages time and other resources to meet established goals within the agreed upon time frames EDUCATION/practice: High School Diploma/GED required 2 years of related work practice.
