Peer Support SpecJOB DESCRIPTIONPOSITION SUMMARYthis job offer will function as a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. this job offer will also act as a resource to the clinicians and case manager, through sharing their expertise of common challenges individuals with SPMI endure and sharing expertise about community resources. This person should be an individual (adult) with lived practice with mental illness. ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned. » Collaborate with service providers involved with individuals with SPMI. » Provide information and peer to peer support to individuals who are served by the program. » Educate and empower clients and their supports regarding their functions in developing crisis plans and in crisis avoidance. » Educate service providers regarding an individual's function in developing a crisis plan for themselves and in crisis avoidance. Ensure that the individual is an integral part of the crisis planning process. » Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-located practices. Maintain and model a consistent focus on strengths. » Advise Clinical manager as needed. » Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need. » Educate individuals and primary caretakers on how to advocate if services are not meeting their needs. » Serve as a liaison to other local supports in the community. » Link individuals to Peer Support Services activities such as support groups, social events, newsletter, etc. managerY RESPONSIBILITIES this job offer has no managery responsibilities. leadership abilities Provides limited guidance and/or consultation to other staff through direction, review and feedback of assigned tasks. SUPERVISION RECEIVED General supervision: routine tasks are performed with minimal supervision; standard practices or procedures allow to proceed alone at routine work; occasional check of work while in progress; work is reviewed upon completion. Work is reviewed by manager for soundness of judgment and overall adequacy and accuracy. This individual should have a working expertise of the OMH Service delivery system and services for individuals with SPMI in the Capital region (Rensselaer, Schenectady, Saratoga, Warren, and Washington counties). COMMUNICATION and CONTACT with OTHERS Requires effective communication expertise. Communication involves interpersonal contacts with clients/families or programs/departments to coordinate service, treatment, or other work. Frequent telephone contact or written correspondence inside the program/department and/or outside the organization involving communication of standard procedures or discussions of non-controversial matters. Regularly training/teaching clients, customers, or new employees in standardized programs or procedures. DECISION MAKING Work is performed under occasional supervision with regular guidance and follow-up. Standard procedures are followed and unusual problems are referred to the manager. Suggestions for decisions and resolutions that are within accepted guidelines are encouraged. CONSEQUENCES of ERRORS Image/Reputation: Errors could cause client related image/reputation concerns, inaccurate reports/records, and could result in dissemination of inaccurate or incomplete information. MATHEMATICAL expertise capability to add and subtract two digit numbers and to multiply and divide with 10's and 100's. capability to execute these operations using units of American money and weight measurement, volume, and distance. TECHNOLOGY expertise Intermediate competency in Windows operating environment using MS Office applications, database, e-mail and internet programs. Work requires over 50% community travel/visits. Community travel/visits require visits to client-related service provision sites.
