RTF Transition CoordinatorPOSITION OVERVIEWPOSITION SUMMARYthis job offer is a member of the RTF treatment team, responsible for working closely with youth, their family, and all members of the team to identify transition and discharge goals for youth. By working closely with all members of the treatment team, the goal is to achieve prosperous results for the youth and family. ESSENTIAL tasks AND RESPONSIBILITIES » Complete selected discharge needs assessment upon client admission to RTF. Assessfamily strengths and needs and assume primary responsibility for the development andcoordination of a service plan to address impediments to family reunification. » Ensure linkage to the community and the involvement of the local child serving system(OMH, DSS, CSE, etc.) in the discharge and transition process for each assigned childand family. » Maintain connection and communication with the family/guardians during the youthsRTF stay. » Involve the family in every aspect of the treatment and discharge process. » Coordinate with RTF treatment team regarding the familys progress, the childstreatment plan and planning for therapeutic home visits. » Assist youth and their families with reunification or with an alternative transition plan. » Meet with the family in the community or in the home as needed. » Provide aftercare contact and assistance with family identified concerns and needs. » Keep written records according to regulatory and programmatic standards.
