Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Health Home Unit manager to join our team at our location in Peekskill, NY.POSITION SUMMARY Health Home Site manager is responsible for the supervision of care management staff and the everyday care management operations of the program. This includes supervision of Health Home staff, ensuring quality of care management services provide to clients, management of staff documentation requirements, and staff training. Also, the site manager will ensure the provision of adequate clinical support to clinically complex clients and clients in crisis. ESSENTIAL FUNCTIONS » Provide direct supervision to care managers to assist with the development of effective interventions and Care plans » Ensure staff is kept informed of Health Home policies and procedure as well as departmental priorities. Receive appropriate trainings in order to maintain clinical standards » Monitor Health Home compliance and quality assurance » Provide crisis intervention to clients in psychological distress » Complete enrollment and service data tracking reports monthly or as needed » Continually reviews the service delivery process to exceed customer expectations. » Review and approve client documentation as assigned » Consistently exhibit New Co.'s core values of team work, open book management, advocacy and customer service » Participate in quality assurance and quality improvement activities » Conduct staff meetings, trainings and supervision » Occasional field work may be required as needed to achieve client results and health home requirements » Performs other tasks as requested » Facilitation of bi weekly Clinical Case Review with the Care Managers EDUCATION/practice: » Master's Degree in Social Work, Psychology or Human Services preferred » One year post graduate practice working with individuals or families who have a history of mental illness, substance use, homelessness or HIV/AIDS » Previous managery practice required » Strong leadership abilities capability » Excellent oral and written communication expertise. » Excellent interpersonal expertise. » Good problem-solving, decision-making and judgment expertise » capability to work in a team management system income Range: $39,130.00 - $42,770.00 Annually Skills:
