Discharge Planner

Ten Broeck Center for Rehabilitation & Nursing is now hiring a full time Discharge Planner to join our team! Ten Broeck is a 258-bed facility that is recognized all over the Hudson Valley for our outstanding delivery of subacute and long-term care.
The Discharge Planner is responsible formanaging and organizing the treatment plan and discharge of an assigned caseload.this job offer initiates individualized treatment plans, acts as an informational liaison between the facility and third-party payers as well as coordinates needed services for a safe and orderly discharge. this job offer works as part of an interdisciplinary team to enhance the quality of the patient's stay. ESSENTIAL FUNCTIONS OF A DISCHARGE PLANNER:
   » Regularly assesses the needs of the patient, interact with all disciplines to ensure those needs are met within the plan of care.
   » Reports all cases of alleged violations involving mistreatment, neglect, or abuse, including injuries of unknown sources.
   » Attends Medicare meetings, subacute meetings, departmental meetings, and all others as assigned by the manager.
   » Responsible for providing therapeutic interactions to patients and families where problems or attitudes may interfere with the patient's care.
   » Assists patients/responsible parties on day of admission with appropriate forms.Ensures that the patient's civil, personal and property rights are protected, and that the patient and responsible party understand these rights.Explains all forms including the Self Determination Act.
   » Completes all documentation timely.
   » Completes the social service section of the MDS+ as assigned.
   » Participates in the interdisciplinary care plan meetings and invites the patient and family to attend.Reports on assigned residents, covering Resident Assessment Protocol Triggers.
   » Acts as a discharge planner.
   » Prepares resident and family for discharge.Provides appropriate services and preparation for an orderly and appropriate discharge. Documents same in the medical record.
   » In case of hospitalization, explains bed retention policy to patient and responsible party verbally and in writing.Document notifications in the chart.
   » Provides consultation to other departments regarding an individual patient or general situation that may impact the well being of the patient.
   » Ensures that information regarding patients is kept confidential.
   » Maintains an knowledge of Department of Health regulations, federal and state statutes and laws that impact the department and residents.
   » Attends department meetings as scheduled.
   » Submits a written monthly report to the Director of Resident and Family Services.
   » Forwards Rugs (Resource Utilization Groups) notification to residents and or designated representative and documents it.
   » Personal Trust Funds.Will contact families if funds are not available to purchase needed items.
   » Refers residents to the Adult Day Healthcare Program where appropriate.
   » Assist with transportation, if needed, to and from activities.
   » Assist resident to participate in individual activities of choice, i.e.: set up of equipment, providing sufficient lighting and space.
   » Inform residents of various activities available to them even when Activities staff are not available.
PHYSICAL REQUIREMENTS OF A DISCHARGE PLANNER:
   » Push a wheelchair.
   » Lift a medical record.
   » Be able to visit each assigned resident/ patient within his or her domain.
   » Visual acuity is necessary to read medical chart and observe residents.
DISCHARGE PLANNER Qualifications required
·Bachelor's degree in a human service field preferred .
·Previous case management and documentation practice preferred. Previous practice in healthcare required.
·Interview and therapeutic relationship expertise.
·Knowledge of community, Human service agencies.
·Knowledge of medical terminology is preferred.
