This essential function focuses on providing responsive, high-quality palliative and hospice care to patients and families across Orange and Sullivan Counties. Ideal for practiced Nurse Practitioners seeking a flexible, primarily remote position with significant impact.

Nurse Practitioner Position Overview
As a Weekend On-Call NP, you will collaborate closely with Hospice Medical Directors and interdisciplinary team members to assess and manage patient needs. This hybrid function allows for remote work, with the potential for occasional in-person visits located on patient needs and organizational priorities.

Nurse Practitioner Responsibilities
   » Provide weekend on-call coverage (most calls occur between 8:30 AM and 3:00 PM)
   » Respond to nursing staff calls and secure messages regarding symptom management and patient care needs
   » Prescribe medications and authorize refills as appropriate
   » Complete required New York State death certificates and hospice documentation
   » Conduct remote or in-person visits for eligibility assessments (Face-to-Face), symptom management, and care collaboration
   » Review clinical records to confirm hospice eligibility and terminal diagnoses
   » Maintain accurate and timely medical records and documentation for regulatory and billing compliance
   » Provide support, guidance, and education to staff as needed
   » Adhere to HIPAA and all policies related to patient privacy, safety, and compliance
   » Maintain licensure, certifications, and DEA registration; participate in continuing education and in-services
Nurse Practitioner Qualifications
   » Master's degree (MS) or Doctorate (DNP) in Nursing
   » Current Nurse Practitioner license in New York State
   » Active RN license and DEA license to prescribe controlled substances in NY
   » Minimum of 2 years’ Nurse Practitioner practice required
   » At least 1 year of hospice or home health practice preferred
   » Hospice and Palliative Care Certification preferred
   » Pediatric care practice a plus
   » Valid NY driver’s license and personal vehicle insurance
