Aging in Place Specialist

JOB DESCRIPTION

We are seeking for someone who is a MA, LPN, RN, BSN or someone in any related field! Is that you?

Do you have practice in the medical field and interested in helping the aging population with staying in their homes while being involved in their medical care? If your answer is yes, the Aging in Place Specialist position maybe a great fit for you!

As an Aging in Place Specialist, you will help to provide an opportunity for the consumer to be more involved in their care plan as well as helping to ensure a continuum of care is provided by healthcare providers and caregivers.

Independent Living, Inc. is a consumer-directed, cross-discapability advocacy and service organization dedicated to enhancing the quality of life for individuals living in the Hudson Valley region and we seek to hire an Aging in Place Specialist to assist consumers with transition and diversion from institutional settings. Flexible hours required located on the needs of the position. Minimum educational requirement is a Bachelor's in Business Management or any related field. tasks AND RESPONSIBILITIES:
   » Complete comprehensive intakes and community and functional assessments that may include, but are not limited to; pressure injury risk, mobility and fall risk, mental health, etc. with consumers, caregivers, and providers and collects appropriate information regarding client status, support system, and need for services for care plan development.
   » On behalf of consumer, meets with hospital or other facility staff to assist with long term care planning.
   » Assists with discharge planning and transition and diversion from institutional settings.
   » Monitor delivery of services and follow-up with members, caregivers, or providers through in person visits and telephonic contact.
   » located on intake and assessment information, coordinates referral for services.
   » Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care.
   » Collaborate with discharge planners, physicians, and other parties to ensure appropriate services are integrated into the discharge plan, care plan, and coordination of acute care and long term care services.
   » Assist member with filing and resolving complaints and appeals.
   » Assists with identifying and obtaining needed durable medical equipment.
   » Assists consumer and care givers with reviewing insurances including private, Medicaid, Medicare, etc.
   » Provides ADA consultation for healthcare facilities and coordination with the municipalities, as well as arranging for and/or performing in-home evaluations to determine accessibility needs and provide recommendations.
   » Manage a case load for people with disabilities and older adults with long term care needs.
   » Monthly and quarterly contact to include both in-person and telephonic support.
   » Must maintain consumer assessments and notes.
   » Monitor delivery of services and follow-up with consumers, caregivers, and providers through in person visits and telephonic contact.
   » Work with MCOs/MLTCs or others and coordinate referral for appropriate services.
   » Respond to gaps in service delivery and identify functional deficiencies in plans of care.
   » Assist in coordinating the development of informal or voluntary services to integrate into the member care plan.
   » Assist member with filing and resolving complaints and appeals.
   » Oversight of Statewide Systems Advocacy Network grant which funds a part time position. Attends meetings for this grant as needed.
   » Other tasks as assigned or as required by the needs of the position.

   » capability to maintain a high level of confidentially pertaining to all aspects of Agency operations.
   » Highly professional with excellent communication expertise, professional attitude, and the flexibility to work alternate hours if needed.
   » Must be detail oriented.
   » Willing to share ideas and work closely with manager and team members to advance program and achieve program goals.
   » Honesty, respect for diversity, high personal standards, language and behaviors reflecting such values and the Independent Living philosophy.

Qualifications

   » Bachelor's in Business Management or any related field.
   » Bi-lingual English/Spanish preferred.
   » Proficiency with Microsoft Office.
   » Valid NYS drivers license and availability to use personal vehicle for work related travel within the Mid-Hudson region with mileage reimbursement, as applicable.
   » Flexible hours required located on the needs of the position.
