Sun River Heath is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Peer Advocate Specialist to support our Peekskill location.The Peer Advocate Specialist provides peer support services located on clinical needs as identified in the patient's treatment and recover plans. Essential tasks and Responsibilities: » Provide peer support services located on clinical needs as identified in the patient's treatment and recover plans. » Assist patients in articulating personal recovery goals through groups and individual sessions. » Peer Advocate will support patients in the identification of their strengths and resources to aid in the achieving treatment goals. » Functions as integral part of multi-disciplinary treatment team. » Identify and utilize established tools to assist patients in developing wellness and recovery plans that are individualized. » Educate and function model the value of individual's recovery practice. » Utilize Motivational Interviewing to effectively engage patients in treatment, recovery and after care services. » Provide community outreach to connect patients and family members to services. » Assist the treatment team in the monitoring of recovery and treatment results. » Facilitate peer support groups. » Model effective coping expertise for patients. » May co-facilitate family network meetings. » Promote patient's advocacy in accessing services in multiple systems. » May attend external meetings with patients. » Provide community resources and linkages to vocational, recreational, social, educational and spiritual resources. » With guidance from other treatment team members, utilize and teach problem solving strategies with individuals and groups. » In collaboration with the treatment team will work closely with patients in deciding steps to achieve goals that are self-directed towards recovery. » May document in the clinical record as required by regulation. Minimum Education Requirement: » High School Diploma or equivalent Minimum Work Related practice: 1-2 years Hourly Range: $16.00-$17.50/hour Profil / Compétences:
