Clinician/Social Worker, Youth ActPOSITION OVERVIEWClinician/Social Worker, Youth ACTBase income - $70,000 Sign-on bonus of $2,500 Relocation Package available within New York State! Do you want to make a difference in the life of a young person? As a Clinician in the Youth ACT (Assertive Community Treatment) program, you have the opportunity to help youth and families with the highest level of need. Youth ACT Clinicians, along with the rest of the Youth ACT team, work to keep youth and families together in their communities by providing them with the expertise and resources to overcome their challenges. Clinicians in this program help children avoid hospitalizations and out-of-home placements by providing intensive, community-located services to these youth and families. Our Clinicians are part of a tight-knit multi-disciplinary team that provides medication treatment, therapy, skill building, and psychoeducational support to youth and families to meet all of their needs. What a Youth ACT program Clinician with Northern Rivers can expect: » Small caseloads and ample supervision and support to provide intensive services to the 36 youth and families that the team serves. » Specialized training in the Youth ACT model as well as the principles of person-centered planning, motivational interviewing, trauma-informed care, social determinants of health, cultural competence, implicit bias, and many other best practices. » A lead function in providing services to clients/families such as: » Risk Assessment » Substance Abuse Screen/Assessment » Suicide Assessment » Mental Health needs assessment » Social Needs Screening - Health Leads » Comprehensive Psychosocial Assessment to include diagnosis and needs to develop a treatment plan (IAP) » Close collaboration with collateral providers involved in the lives of the youth and family » Opportunities to provide holistic care to the youth and family system via individual, group, and family counseling modalities.
