At Northern Rivers Family of Services, we work together to make a difference in our client's lives while also strengthening our community.POSITION OVERVIEW Title: Nurse - RN Program: Assertive Community Treatment (ACT) Status: Full-Time Schedule: Monday-Friday 9AM- 5PM 375 Bay Road, Queensbury, NY Sign-on Bonus of $5,000 We are seeking for a talented Registered Nurse to meet the needs of people within the community who have serious mental illness diagnosis and challenges. The Full-time Registered Nurse We are pursuing for the Assertive Community Treatment program would be located in Queensbury, NY. You will work with a team of dedicated professionals providing care and treatment to the clients we serve. As an RN on staff, you should feel comfortable communicating with families, providers, and social workers in order to meet the needs of this population. ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned. » Conduct metabolic monitoring (tracking blood pressure, weight, and body mass index) for participants. » Obtain and ensure processing of lab work for patients unable to have lab work done elsewhere in the community. » Obtain lab results when participants have labs drawn by outside providers. » Administer depot injections to patients on - site and in the community. » Coordinate clozapine monitoring (obtaining lab results, reviewing them, and faxing prescriptions as needed. » Assess responses to medications and symptoms » Assess for medication side effects and educate participants on how to manage side effects » Work with participants and families to collaboratively address medication issues » Coordinate care for medical issues, including providing referrals to community medical providers, scheduling appointments, and liaising with medical providers to obtain records. » Conduct quarterly health screenings. » Provide wellness counseling and education covering issues including healthy eating habits, exercise and fitness, stress management, birth control, and safer sex. » Provide basic first aid and managing medical emergencies and crisis situations appropriately.
