JOB DESCRIPTION

Case Manager opportunity supporting adoptive and guardianship families in the Capital District
Northern Rivers' Permanency Resource Center helps families formed through adoption and guardianship find necessary resources for the variety of issues and needs that arise after finalization. The Permanency Resource Center supports adoptive and post-guardianship families, empowering them for growth and enabling positive results for all involved. Our workers have broad knowledge and lived practice parenting children with emotional, mental health, and trauma-located needs.
Case Manager - PRC
Full-Time - Hybrid
Monday - Friday days with some early evening flexibility needed located on client family needs.
   » Candidates with a bachelor's degree in social work, psychology, or human services who are interested in entering the field are highly encouraged to apply.
   » candidates with bachelor's degree or an associate's degree in other disciplines that also have a minimum of two year of case management will also be considered for the function.
   » As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. You will report to the Case Manager manager.
   » The main office location for this function is 401 New Karner Road, Albany, NY
   » The Regional PRC Case Manager will be assigned various families within the 8 county region although the expectation for home visits is 3-5 total per week.
   » You will work in a hybrid environment with sometravel to client's homes in the community. (Approved travel related expenses reimbursed at predetermined rates)
Responsibilities:
   » Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.
   » Meet with families in their home to assess needs
   » Assess needs, conduct service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment and review with the family via in-home or community monthly visits. Services include but are not limited to information and referral services; referral to legal services; attendance at family court dates; transportation; housing assistance; childcare; basic family needs (vital needs in emergencies and with expenses that are unforeseen); educational advocacy services; and facilitating appointments with social services, medical health providers, and mental health services.
   » Respond to the request for in home support referrals for resources and services
   » Provide crisis intervention supports when needed
   » Work with family to access needed respite services
   » Conduct follow up contacts with families at prescribed intervals
   » Participate in family engagement activities
   » Co-lead support groups
   » Participate in training programs offered by the program as assigned
   » Use lived practice to empathize and provide hope. Preference for someone with practice with kinship, adoption or guardianship as a youth or in a parenting function.
   » Maintain records including required data collection information according to the program standards and expectations. Case Managers must maintain Evolv record with progress notes and all pertinent documentation, a discharge summary and post-case closure documentation.
   » Participate as a trainer in programs in the community, to increase knowledge of the needs of families who have adopted or have children in guardianship or kinship care
   » Complete risk assessments as required by Northern Rivers Quality Management Department
Requirements and suitable education
   » Bachelor's level degree in social work, psychology, or human services, or a bachelor's level degree in any field with at least two years of case management practice; or an associate's level degree in psychology or human services with at least two years of case management practice.
   » Must be able to communicate effectively and develop positive working relationships
   » A valid NYS drivers license
