Clinician - CORE Services CORE services are a collection of services for HARP eligible individuals with a serious mental illness that are community located. Community Psychiatric Support and Treatment (CPST) includes goal-directed supports and solution-focused interventions with the intent to achieve person-centered goals and objectives. This is a multi-component service that consists of therapeutic interventions such as clinical counseling and therapy, which assist the individual in achieving stcapability and functional improvement. CPST addresses behavioral health barriers that impact everyday living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community participation. this job offer involves traveling across Ulster County. In this job offer, the clinician may provide services via telehealth, in the office, or in person at the individual's residence, depending on the individual's preference. There is the option for a remote/hybrid schedule. expertise Desired: Assessment strategies Engagement strategies Teaching Individuals How to Manage Behavioral Health Symptoms Identifying activators Crisis avoidance Working to avoid the identified precursors Supportive Counseling and Focused Interventions expertise of evidence-located practices Goal planning expertise of community resources Supporting active community engagement expertise and Appropriate Diagnosis of Major Psychiatric Diagnoses In addition to providing clinical services, the clinican is expected to: Maintain timely documentation in the agency EHR Completed all required trainings Attend staff meetings as required Communicate and collaborate with other providers and staff members Attend supervision as scheduled Develop ISP's with the individual that are person centered This is not an exhaustive list and responsibilities may change dependent upon agency and program needs. Licensure Required: NYS license: LMHC, LMSW, LCSW practice: practice providing clinical direct services to individuals with severe and persistent mental health and/or substance use disorder.
