Responsibilities Responsibilities: » Assesses the environment of the home and community to identify elements which impact negatively on the patient/family capability to function. » Provides direct short-term counseling services to patient/family or makes appropriate community referrals. » Teaches coping expertise to patients/families to assist them in their adjustment to the life changes connected with illness/discapability. » Participates in conferencing and planning with other disciplines caring for the patient. » Prepares clinical progress notes and documents in the patients record, as indicated. » Available for planned social work consultation on patient care with staff, as needed. » Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.
