We are seeking for the right individual for our Children's Health Home Care Manager position, which covers the Saratoga/Warren/Washington counties!
The Care Manager position supervises the care of the child so that all their needs are addressed and met in a comprehensive manner to stay healthy and out of the hospital. They do this bymaintaining a caseload of children, their families and other caregivers, and meeting with each on a monthly basis.
As part of a care-located treatment team, this job offer will identify and discuss the strengths, risk factors and service needs that may impact the health and safety of the client and family, as well as develop and document progress on treatment goals and methods.
Your Education and practice:
» Bachelor's degree in Social Work or in a related field.
» Two years of practice providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse.
» Requires capability to drive to home visits with clients: Must possess a valid driver's license and vehicle.
