JOB DESCRIPTION

POSITION SUMMARY

this job offer will be responsible for the delivery of the services assigned, and completion of related documentation, in the Tier 1 and 2 arrays. Tier 2 Services CSP-2Bs are eligible to provide include:
   » Children and Family Treatment Support Services o Community Psychiatric Supports & Treatment (non-clinical services only unless
EBP certified) o Family Peer Support Services (if lived practice criteria is met) o Youth Peer Support Services (if lived practice criteria is met)
   » Home and Community located Services o Community Self-Advocacy Training and Supports

ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned. Employees are expected to be awake and alert while working.
   » Provide services located on the child and caregivers needs and preferences as identified in the Treatment Plan
   » Coordinate with the child and caregiver on scheduling and provision of services
   » Work with family members, and salient parties to identify goals and objectives of treatment
   » Complete all required documentation and submit to the manager within required timeframes
   » Facilitate service provision and attend significant meetings as required
   » Share information related to delivery of services with stakeholders as required
   » Provide comments and information to collateral contacts for the purpose of Treatment planning, assist in tracking progress
   » Attend all required trainings
   » Meet regularly for supervision
   » For Full Time staff members 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan

SUPERVISION RECEIVED

General supervision: routine tasks are performed with minimal supervision; standard practices or procedures allow to proceed alone at routine work; occasional check of work while in progress; work is reviewed upon completion. Work is reviewed by manager for soundness of judgment and overall adequacy and accuracy.

EDUCATION and/or practice
Requires a Bachelors degree in a related field and at least two years of related practice.

CERTIFICATION / LICENSURE
Requires a valid drivers license.

FINANCIAL MANAGEMENT RESPONSIBILITY

Responsible for limited financial transactions. May be responsible for petty cash, flex funds, or similar types of financial transactions.

MATHEMATICAL expertise

capability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. capability to compute rate, ratio, and percent and to draw and interpret bar graphs.

TECHNOLOGY expertise

Intermediate competency in Windows operating environment using MS Office applications, database, e-mail and internet programs.

WORK ENVIRONMENT

Work requires 25 50% community travel/visits.
Travel requires use of own vehicle in transporting clients.
Community travel/visits require visits to client homes.
Possible exposure to blood borne pathogens. Possible exposure to verbal and/or physical aggression.
