Activities LeaderTen Broeck Center for Rehabilitation & Nursing is now hiring a part time Activities Leader to join our team! Ten Broeck is a 258-bed facility that is recognized all over the Hudson Valley for our outstanding delivery of subacute and long-term care.The Activities Leader is responsible for carrying out the Activities program designed to meet the individual needs of the residents in facility. They keep continuing and accurate progress notes and attendance records of all residents to and from activities, and give input into monthly activity calendars for the units. (i.e.: religious services, physical ad cognitive games, arts and crafts, gardening, grooming etc. » Communicates with residents with visual, speech, auditory, cognitive, physical and emotional impairments. » Completes Activities section of MDS forms as scheduled. ·Regularly assess the social and emotional needs of the residents with the Director and set appropriate goals for meeting those needs within the overall plan of care. » Helps in keeping activity materials organized. » Helps decorate the facility with seasonal decorations. » Gives input on monthly activity calendars and hangs them on appropriate units. » Works with the interdisciplinary team, attending various weekly care plan meetings. » Reports all cases of alleged violations involving mistreatment, neglect or abuse, including injuries of unknown sources. » Assist with transportation, if needed, to and from activities. » Assist resident to participate in individual activities of choice; i.e.: set up of equipment, positioning assistance, providing sufficient lighting and space. » Assures safety of residents at all times. » Other tasks as requested. PHYSICAL REQUIREMENTS OF AN ACTIVITIES LEADER: ·Transporting residents to and from in -house activities and accompanying residents to and from community programs. ·Position requires movement all over the entire building. » Pushes 225 lbs. in a wheelchair, recliner, or Geri - chair. » Leading appropriate exercise classes (Pushing, bending, stretching) ACTIVITIES LEADER Qualifications required ·High School Diploma or GED required. » One-year practice in providing activities or practice in a health care setting preferred. » Documentation practice. » practice/capability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
