Clinical Case ManagerJOB DESCRIPTIONClinical Case Manager, Intensive Family Empowerment ProgramAs a Clinical Case Manager in the Intensive Family Empowerment Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and will work in a combination of on-site and remote settings with travel to client's homes required. Responsibilities: » Provide home and community services to the identified youth and their caregiver as prescribed in the treatment plan for those enrolled in our Family Preservation Services. » Educate parents and their child in a variety of social-emotional and parenting expertise such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc. » Maintain a caseload of three children. » Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, reports, etc. following agency, county, and state policy and funding requirements. » Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews. » Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment. » You will work Monday through Friday with a flexible schedule that includes evenings and a rotating on-call schedule. Requirements and suitable education » Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field; » practice working with at-risk youth and families; » Home and community-located practice is a plus.
