Expected Start Date: October 27, 2026 Union: SUN Facility: Weyburn Primary Health Care City/Town: Weyburn Department: Primary Care Sites Type: Full-time temporary Expected Up to Date: October 15, 2027 FTE: 1 Shift Information: Days Number of Hours per Rotation: 28 shifts of 8 hours per 6 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: Yes Job Description: Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care. PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills. Human Resources Exemption: No Experience Experience in teaching adults, facilitation skills, and leading group activities Minimum one year of recent nursing experience in a clinic-based setting required Education Bachelor of Science in Nursing (BScN) Other Education and Training Basic Life Support Certificate (BLS) Nursing diploma is acceptable Competencies Advanced - Organizational skills Advanced - Computer skills Intermediate - Leadership skills Advanced - Time management skills Knowledge and Abilities Demonstrates ability to accept and implement change Demonstrated understanding of the prevention and management of chronic diseases Knowledge in chronic disease prevention and management Knowledge in LEAN management system Ability to work effectively with individuals who are First Nations or Metis Knowledge of the impact of social/economic/cultural issues on chronic disease Proven conflict resolution skills Demonstrates effective verbal and written communication skills. Demonstrates empathy, trust and respect in the provision of care Knowledge of group dynamics and facilitation skills. Knowledge and application of quality improvement principles, tools, and outcomes Ability to work independently and as a member of a multi-disciplinary team Makes independent clinical judgment based on evidence based best practice Competent critical thinking/judgement/autonomous decision making capabilities Ability to work under pressure and establish priorities Ability to use health promotion and prevention strategies for program planning Other Information Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS) May be eligible for Recruitment and/or Retention Incentives Experience in case management in health care setting is an asset Experience in a community-based setting (Public Health, Home Care) is an asset.
