Job Description:
Including but not limited to:
Creating, updating and reviewing Care Plans

Documenting LTC-F

Scheduling and facilitating care conferences

Process Admissions/Discharges

Assisting with Outbreak procedures

Assisting with Physician rounds

Assist with ordering medical supplies

Reporting to Infection Control
